I acknowledge that I have voluntarily chosen to participate in a chair yoga class provided by New Sun Wellness and its affiliates I understand that the services provided can involve physical movement and exertion and that there are risks and dangers inherent in any physical activity. I certify that I am in good physical health and that I am able to participate in the chair yoga class. If I have any medical conditions or concerns, I will consult with my doctor before participating in the class. I understand that it is my responsibility to notify the instructor of any health conditions or concerns that may affect my ability to participate in the class, and to follow the instructor's guidance and instructions during the class. I hereby release and discharge the yoga studio, the instructor, and their respective owners, agents, and employees from any and all claims, liabilities, and damages arising from my participation in the chair yoga class, including but not limited to any injury, illness, or loss of personal property. I understand that this waiver is intended to be as broad and inclusive as permitted by law and that if any part of it is held to be invalid or unenforceable, the remainder of the waiver will continue in full force and effect. By signing this waiver, I acknowledge that I have read and understood it, and that I voluntarily agree to its terms. By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.
